To be officially registered as a member of St Alphonsa Syro-Malabar Parish, Brisbane North, the following requirement must be submitted:
An official letter or certificate issued by your former Parish Priest stating:
- Your Full name
- Your family name (house name)
- Your marital status (married / single)
- If married, the name of your spouse
- Confirmation that you were a member
This letter must be:
- Printed on official parish letterhead
- Signed and sealed by the Parish Priest
- Submitted either as a physical copy or verified digital copy
